  • Overview

    This position is located in the National Disaster Medical System (NDMS), Division of Emergency Management and Medical Operations (EMMO), Office of the Assistant Secretary for Preparedness and Response (ASPR) in the Office of the Secretary, US Department of Health and Human Services (HHS). The purpose of the NDMS is to support State, local, Tribal, Territorial authorities following disasters and emergencies by supplementing health and medical systems and response capabilities. NDMS would also support the military and the Department of Veterans Affairs health care systems in caring for combat casualties, should requirements exceed their capacity. In this role, one would develop and implement a robust clinical case management program to directly support patients under the NDMS congressionally mandated Definitive Care program.

    Responsibilities

    Be experienced in developing a tracking system for patient care coordination and care management across the continuum. Experience in independently planning and providing nursing care based on professional nursing practices and medical protocols or guidelines for patients. Experience in collaborating with federal and non-federal partners, subject matter experts (SMEs) to develop clinical protocols and guidelines that establish standards of care. Experience in providing case management for patients by coordinating services to meet patient's care goals. Experience in providing oversight to ensure compliance and appropriateness of care.

     

    Develop and implement a robust and comprehensive clinical case management, care coordination, and discharge/disposition services across the NDMS platform. Will develop an evaluation process to evaluate clinical case management activities and outline a process to ensure ongoing monitoring and evaluation of the Clinical Case Management Program functions within established professional guidelines using objective criteria.

     

    Establish and facilitate collaborative clinical case management processes, which assesses plans, coordinates, implements, monitors, and evaluates care options and medical resources/services, to meet complex health needs across the care continuum with the goals of decreasing fragmentation of care and enhancing the patient’s quality of life while containing costs during the active participation within the Definitive Care program. Employ a holistic approach considering diagnosis, prognosis, pathophysiology, physical, mental, psychological, and emotional needs, demographics, and home environment to provide the optimal level of care and patient outcome.

     

    Develop and implement NDMS wide case management screening to include, but is not limited to: individuals with acute, chronic, multiple, complex, catastrophic, life threatening illnesses/injuries and/or high medical services utilization rates; patients that are non- compliant with their care regimen; and those patients who warrant extended services following discharge. Will coordinates with members of the federal government partners, non-federal government partners, NDMS partners, health care facilities for clinical case management when NDMS patients require additional care outside a designated mission assignment.

     

    Develop standard operating procedures, directives or guidelines by researching and reviewing fundamental case management processes and recommending procedures to enhance the timeliness and quality of case management services to NDMS patients.

     

    Develop and implement clinical case management care plan guidelines for NDMS patients. Advise leadership of solutions to current and future requirements and consults with staff at various levels of the organization to address complex, sensitive case management challenges to develop strategies that improve NDMS patient outcomes, patient/family satisfaction with services and overall cost effectiveness.

     

    Serve as the lead of the interdisciplinary healthcare team to ensure appropriate coordination of care to enhance patients’ health and wellness, safety, productivity, and quality of life. Establishes processes for active collaboration/participation with Health Care Integrators, Utilization Managers, Discharge Planners, Disease Managers, and NDMS partners clinical staff in unit level Interdisciplinary Patient Care Conferences to effectively provide patient case management.

     

     Coordinates with other healthcare professionals/departments to develop an effective process for collecting/evaluating utilization data, monitoring performance, identifying opportunities for improvement, and initiating performance improvement programs to enhance patient outcomes.

    Qualifications

    Required Education:

    Must have a degree or diploma from a professional RN nursing program approved by the legally designated State accrediting agency at the time the program was completed by the applicant. Must have active, current registration as a RN in a state, District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.

     

     

    Required Experience:

    At least two years of recent experience in nursing case management/health care management for adults, children, families, seniors, or groups is desired.

     

     

    Required Clearance: Public Trust

     

     

    Desired Qualifications: 

    -Must maintain competency in Basic Life Support (BLS) certification.

    -Skilled in analyzing data defining health care needs of a defined population using standardized assessment tools and operating computer programs and databases such as Excel, Access, Word, etc.

    -Ability to apply critical thinking skills and expertise in resolving complicated healthcare, social, interpersonal and financial patient situations.

    -Ability to effectively communicate orally and in writing with a diverse group of individuals.
